Credit Card Authorization Form
A 25% deposit or $100 minimum is required to book a wedding. All deposits are non-refundable. If plans should change, your deposit is transferable up to one year from date of deposit. Final payment for your wedding is due 10 days prior to your wedding date and will be charged to your credit card unless prior arrangements are made. If you cancel 5 or more days prior to your wedding all monies are non-refundable but may be transfered to a future date within one year of your original deposit date. For a cancellation less than 5 days prior to your wedding, all monies are non-refundable and non-transferable.

Minister Fee not included - $60.00 Minimum. Customary Photographer tip not included. No Cameras allowed on premises. All flowers and other wedding accessories are to be provided exclusively by Belleza Wedding Chapel unless other arrangements are made.

I have given my credit card number to Belleza and I understand that this transaction is non-reversible. I understand that my credit card will be charged the above deposit amount and is non-refundable. Non-refundable deposits are 25% of the total cost of my wedding package or $100.00 whichever is greater. I authorize Belleza to charge my credit card the remaining balance (10) ten days prior to my wedding date. I acknowledge that all monies are non-refundable 10 (ten) days prior to my wedding date, unless I provide written cancellation to Belleza (10) ten days prior to my wedding date.
